How do I get a lifetime license for my dog?

Lifetime licenses are a great convenience – and money saver – for Pennsylvania dog lovers.

To get one, your dog will need permanent identification in the form of a microchip or tattoo.

When your dog is microchipped, ask your veterinarian to fill out the Permanent Identification Verification Form (PIVF). If your dog is already chipped, your veterinarian can scan the chip and complete the form.

Send the PIVF with the lifetime license application to the county treasurer’s office. It’s that simple.

If your veterinarian doesn’t have the PIVF and license application on hand, stop by the county treasurer’s office at the courthouse to pick them up.

If you’d rather have your dog tattooed, visit the courthouse first to pick up the PIVF with the tattoo number assigned to your dog.

After your veterinarian tattoos your dog, return the completed PIVF and lifetime license application to the treasurer’s office.
